Shisa Cup U11 Cross-Ice Hockey Tournament · Version 2026-10-25
In consideration of the registered player(s) being allowed to participate in the Shisa Cup U11 Cross-Ice Hockey Tournament at Southern Hill Ice Arena, Okinawa, Japan, on February 6–7, 2027 (the "Tournament"), I, the parent or legal guardian of the player (or, for a team registration, the team's authorized contact), acknowledge and agree to the following:
- I understand that ice hockey is a physically demanding sport that involves risks of injury, including but not limited to minor injuries, serious injuries, and death.
- On behalf of myself and the player, I voluntarily assume all risks associated with the player's participation in the Tournament, including but not limited to falls, collisions with other participants, being struck by pucks or sticks, equipment failure, and any other hazards that may arise.
- I am solely responsible for any family members or guests who attend with the player. I agree to inform them of the inherent dangers of the rink environment, including but not limited to flying pucks, collisions, and other hazards, and I acknowledge that they cannot hold the rink or any of the Released Parties (defined in item 4) liable for any injury, damage, or loss that may occur.
- On behalf of myself, the player, and our heirs, I hereby release, waive, discharge, and covenant not to sue Michael Lam, Kung Pow Kings, John Andrews, David Leon, Andrews LLC, the tournament organizers, sponsors, volunteers, or any other participants in the Tournament (the "Released Parties") for any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the player, me, or my guests while participating in or attending the Tournament, whether caused by the negligence of the Released Parties or otherwise.
- This waiver and release of liability is binding upon me, the player, our heirs, executors, administrators, assigns, and any guests I invite, and is governed by the laws of Japan, where the Tournament takes place.
- I certify that the player is physically fit to participate in the Tournament and has not been advised otherwise by a qualified medical professional, and that I have listed in this registration any allergies or medical conditions the organizers should know about.
- If the player is injured or becomes ill and I cannot be reached, I authorize the tournament organizers to arrange emergency medical care for the player, and I accept responsibility for any resulting costs.
- If I am registering a team, I confirm that I am authorized to do so, that the parent or legal guardian of every player on the roster has been informed of and agrees to this waiver, and that I am responsible for my team's adherence to these conditions, including any guests associated with my team. The tournament organizers are not liable for my team's actions.
- This waiver is provided in English, Japanese, Traditional Chinese, and Simplified Chinese. If the versions differ, the English version prevails.
